During the 2008 Health Care Reform, the Minnesota legislature expanded on a national trend of converting clinics to Health Care Homes. Four years later the Department of Health has certified 134 clinics across the state. The legislation includes payments to primary care providers to improve coordination of care and treatment of chronic health conditions by shifting to a team approach.
Dr. Paul Erickson of Northpoint Health Center says it gives the primary care provider more flexibility to evaluate what is best for the patient and for the community.
